Older adults’ use of, and satisfaction with, electric powered indoor/outdoor wheelchairs
BACKGROUND: Research documenting the experiences of electric power wheelchairs indoor/outdoor power wheelchairs(EPIOC) users has generally failed to take into account the specific needs and concerns of older adults. This study sought to qualitatively examine the older EPIOC users’ satisfaction with the chair and service providers. METHOD: Eight women and nine men aged between 60 and 81 (mean 69) years were recruited through a specialist wheelchair service database. All had severe mobility disabilities. Analysis was performed using a qualitative framework approach. RESULTS: Participants reported a variety of EPIOC uses, including shopping and some social contact. Moderate satisfaction with the chair was reported. Use was compromised by indoor and outdoor environmental barriers; and the chair not meeting users’ needs. Accidents were rare, but many users still experienced insecurity in the chair. High levels of satisfaction with the service were reported, although concern was expressed over length of waiting times. CONCLUSIONS: EPIOCs proved useful to most older people with disabilities. However, even those who were satisfied reported only moderate use of the chair outdoors. Limited use related to an infrequent need for outings, outdoor barriers, feelings of insecurity over chair safety and lengthy waiting times for chair delivery and required modifications. Service providers should be aware that older EPIOC users may require extra support and the provision of timely adjustments to increase chair use.

Distribution and cost of wheelchairs and scooters provided by Veterans Health Administration
During fiscal years 2000 and 2001, the Veterans Health Administration provided veterans with more than 131,000 power wheelchairs and power wheelchairs at a cost of $109 million. This national study is the first to investigate Veterans Health Administration costs in providing wheelchairs and scooters and to compare regional prescription patterns. With a retrospective design, we used descriptive methods to analyze fiscal years 2000 and 2001 National Prosthetics Patient Database data (cleaned data set of 113,724 records). Wheelchairs were categorized by function, weight, and adjustability options for meeting individual needs (e.g., axle position, camber, position of wheels, tilt, and recline options). Results displayed a cost distribution that was negatively skewed by low-cost accessories coded as wheelchairs. Of the standard manual wheelchairs, 3.5% could be considered beyond the customary cost. Regionally, 71% to 86% of all wheelchairs provided were manual wheelchairs, 5% to 11% were power wheelchairs, and 5% to 20% were scooters. The considerable variation found in the types of wheelchairs and scooters provided across Veterans Integrated Service Networks may indicate a need for evidence-based prescription guidelines and clinician training in wheeled-mobility technologies.

Reliability and validity of the power-mobility community driving assessment
The Power-Mobility Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or power wheelchairs in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices.

DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and power wheelchairs significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. CONCLUSIONS: The results suggest that because the resultant force at the pushrim has a greater tangential component and a greater proportion of the maximal voluntary force, most of the net moments around the shoulder are higher. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

In 1999, approximately 6.3 million adults3.1% of the Nation’s adult populationwere under correctional supervision (that is, incarceration, probation, or parole).1 Additionally, 98,913 juveniles (9% of whom were drug offenders) were incarcerated in public or private juvenile facilities for nonstatus offenses.2 Drug offenders accounted for 21% (236,800) of the State prison population in 1998, up from 6% (19,000) in 1980,3 and 59% (55,984) of the Federal prison population in 1998, up from 25% (4,749) in 1980.4 Also, in 1998, an estimated 26% (152,000) of all inmates under local supervision were incarcerated for drug offenses.5 This increase in the drug offender prison population mirrors the steady increase in arrests for drug offenses. The Federal Bureau of Investigation (FBI) reported 580,900 arrests for drug offenses (5.6% of all arrests) in 1980. The number of arrests peaked at 1,559,100 (10.4% of all arrests) in 1997. In 1999, there were 1,532,200 drug arrests, which accounted for 10.9% of all arrests.
In 1997, 82,646 and 993,365 male inmates were in Federal and State prisons, respectively, and 6,426 and 66,242 female inmates were in Federal and State prisons, respectively. Women in State prisons were more likely to report using drugs in the month before their offense (62% versus 56%). Women in State prisons (40%) were also more likely then male inmates (32%) to have committed their offense under the influence of drugs. In Federal prisons, male inmates were more likely than female inmates to report regular drug use and drug use a month before their offense. The percentageage cost per year to incarcerate an inmate of male (23%) and female (19%) Federal prison inmates reporting drug use at the time of their offense was about equal.7 In 1998, 65.5% of convicted jail inmates had used drugs regularly and 16% had committed their offense to get money for drugs. More than one-third (35.6%) of jail inmates had committed their offense under the influence of drugs.8
BJS reports that 24% of the adults on probation in 1998 were sentenced for a drug offense, up from 21% in 1995. In 1995, almost all probationers had one or more special conditions to their probation (such as fees, fines, drug testing, drug or alcohol alcohol drug rehab malibu, or community service). Power mobility driving training for seniors: a pilot study
This article describes two power wheelchairs mobility training protocols used with seniors and compares posttraining driving performance. Twelve users of power mobility were consecutively recruited from two residential facilities in Toronto, Canada. The aim of training at both sites was to make clients comfortable with and safe at driving power wheelchairs devices. The content of training was similar, but training protocols differed significantly in terms of the number of sessions (means of 3.43 vs. 9.80; p < or = .05) and the time frame over which the sessions were offered (means of 1.57 vs. 5.10 weeks; p < or = .01). Participants at the two sites differed significantly in terms of overall driving performance (p < or = .05), gender (p < or = .01), and type of device used (p < or = .05). Overall, driving performance was significantly associated with facility, gender, type of device used, and training duration (p < or = .05). When these variables were entered into an exploratory hierarchical regression, facility accounted for 64% of the variance in driving performance. When facility was controlled for, the correlations between device and duration of training with driving performance were no longer significant. The determinants of driving performance are difficult to clearly specify as the variable facility encompasses gender as well as all other differences between the two training protocols. Nevertheless, these data provide direction for future research in this area.
